Please advise us:

Insureds Name & Address of all locations the business operates from. The year the business was established. Commencement date for cover. Name of current Public & Products Liability insurer if applicable. Description of insured's activities. Does the insured operate any owned or non-owned unregistered vehicles beyond the limits of a road. If yes, list the type and number of such vehicles operated. Number and type of other vehicles operated eg. 6 Prime Movers, 2 Rigid Trucks. Does insured use sub-contractors or labour hire firms. If yes, disclose amount of renumeration paid to them each year. Sum Insured required eg. $5M or $10M, or $20M. Excess required. i.e. $500, or $1000, or $5000 property damage only. Disclose all claims made by insured, or incidents that may have given rise to a claim, under Public or Products Liability Insurance in the last 7 years. Disclose details of any declinature to accept or renew or special terms imposed on, a Public or Products Laibility policy held by the insured in the last 7 years. Other matters relevant to the Insureds Duty of Disclosure.
